Choose Health Process
Amaze has a contract with a company called Choose Health. Choose Health is an at-home lab service that tests preventative and wellness type labs using a very small blood sample. The lab results are then sent to Amaze to review and release to the patient. Our Amaze providers will review the labs and either release them without comment (if they’re normal,) or release them with a comment (if the labs are abnormal).
Occasionally, labs will come back with a critical value, OR a patient will request a consult with one of our providers. Critical lab values require a consult with our providers, so from the Patient Advocate perspective, they’re treated the same. When either a consult or critical value comes through, it is the Patient Advocate Team’s responsibility to reach out to the patient to schedule the consult.
The Choose Health Board is a part of our Daily Operations workspace in Monday.com. It has 4 groups, “New Assignments,” “Consults,” “Critical Results,” and “Completed Assignments.”

Labs first come into the “New Assignments” group of our “Choose Health Labs” Board in Monday, within the Daily Operations workspace
Providers will then mark what type (1 – Type) of lab it is, and what action (2 – Status) needs to be taken:
1. Released with or without comment will complete the Task and move it down into Completed Assignments. Nothing needs to be done from the Patient Advocate Team in this scenario.
2. “Schedule Consult” will move the task to “Consults” group and will automatically assign the Patient Advocate Team as an owner of the task
3. “Critical – Contact Patient” will move the task down in the the “Critical Results” group and will automatically assign the Patient Advocate Team as the owner of the task
4. Closed: Critical and Closed: Consult, will close a critical or consult AFTER the consult has been completed by the provider
5. New – Unread will not move the task.
Please note, when a Consult Request comes through, it will link to the Patient’s labs in the update of the task, however, we are required to put a password in order to view the labs. The CH password is: SP@Ac3X!N02I4i and can be found on the Choose Health Board under the tab called “CH password.” Critical results do not require a password.
Once a provider has moved a task into either Consults or Critical Results, Patient Advocate Team is assigned and will be notified. At that point, one person from the patient advocate team is responsible for creating the patient’s account, adding the results to the patient’s account, reaching out to the patient and scheduling a time to meet with one of our providers. We are required to try to reach out to the patient FIVE times, THREE of which must be phone calls. The other two can be either an SMS or email.

2. Add results to chart
3. Communicate with patient to schedule an appointment
4. Look at Shifts on Teams to find which provider will be available at the time the patient requests
5. Put the patient on the Member.Services Outlook Calendar, then invite the provider to the meeting
