There are a lot of templates for SOAP notes out there, please only use the two below for your notes. The Fullnote is for initial PCP visits, the Basicnote is for daily visits. As we advance our technology we will be adding clicks in the portal for these but we are using these for now to keep it uniform and help our fellow providers and advocates be able to quickly peruse our note and find info they need if we are off shift.

Please remember that every call needs a note. Portal call, teams call or, rarely, visit over message .

If you did a visit through messages, please click “Add note” and put in

Please get in the habit of using the templates as they are built to remind you to input certain data. While on the phone, try to verify an up-to-date medication and allergy list, and before moving on to another patient or task, please chart your assessment and plan. Even if you have to come back to finish the rest of your note a few minutes later, please make sure to input your assessment and plan so that anyone coming behind you knows the game plan. We oftentimes have patients or pharmacies call back in, and we want to make sure anyone answering the phone can clearly see what medications were called in and to what pharmacy, etc.
