In order to get everyone set up on our new e-prescribe system, we need your current “practice address.”
For me, it’s my home address, so if that’s where you practice from, that’s fine too. The address will not be shared with patients and will not be public record. I will add the address to our e-prescribe system, so you can select it when you log in as the address you are sending a script from. They will be on file, in case a pharmacy or regulating agency asks for them, but otherwise, will not be shared.
If you have multiple state licenses, please also send me the address associated with each one.
