Handing Off Referrals

Here are the steps on how to appropriately send a request for a referral to the A-Team (patient advocates). To streamline this process a bit more, please follow the steps below for referrals.

IF you already have a task open on the patient (regardless of Daily Management or Task Management Board):

  1. Duplicate the task
  2. On the NEW task: add the Team, “Patient Advocate Team” (if on Task Management board) as an “owner,” and then make sure the column “team” is also Advocate
  3. Label the new task appropriately, i.e. “Urgent Referral – Cardiologist,” or “Routine Referral – Endocrinology,” or “STAT Referral – MRI”
  4. If you have a STAT referral, the best practice is to create this on the Daily Management Board and use the action “Next Advocate” to put it in the “Priority” Group.
  5. In the update section of the NEW task, please include all the information needed for the referral. The easiest way to do this is by using the TextExpander snippet new.referral
  6. Attach a prefilled-out referral form to the task
  7. Finally, add yourself as an “other” on the NEW task. If you do not already have a task open on this patient, please create a new task on the Daily Management board (either in the inbound communications group or Today’s Tasks)
