When a patient has Champion Health – you’ll see this reflected in their chart by Activation Code and Sub.
Every Champ patient should have a “Member ID” in the chart. This is their champion health ID. They will need this ID to log into their Health Wallet
The Health Wallet is a Mobile App that Champ patients should all download, giving them access to their benefits, Rx cards, and network search.
From the App Store it looks like this:
Once in the App, you’ll input Member ID (Found in Portal chart), and Date of Birth
Once you enter Member ID and DOB, you’ll see the following:
“Benefits” Will open up their Insurance ID Card
“VerusRX” Will open up their Rx Card
“First Health Provider” is the Champ Network Search
