Champion Health – Basics

A lot of our patients will have “The Champ Plan” offered by a company called Champion Health. This is a plan that employers offer their employees, which allows employees to use pretax money to pay for certain medical expenses. This falls under Section 125 of the IRS Code (also called a “cafeteria plan”). Amaze is the preventative health/telehealth provider embedded in these plans, and therefore, we are automatically available to all patients who have a Champ Plan.

The other/outside health benefits offered under the plan are limited, but it financially benefits both employer and employee and ultimately decreases an employee’s taxable wages.

Some patients will ONLY have Champion Health, and therefore will lean heavily on Amaze for health services. Other patients will have Champion Health alongside another, more comprehensive insurance plan. Always clarify if a patient has another plan.

Champion Health covers preventative visits with a Primary Care Provider, and Urgent Care visits 100%, as long as patients use an in-network provider. It does not, however, cover many procedures, labs, or imaging done at the PCP, or UC office (only the actual visit cost). Champion Health also has a list of 0$ copay and $1 copay medications.

Specialists, labs, procedures, hospital stays, surgery, and medications outside of their formulary are NOT covered.
