Champ Unenrollment Process
Occasionally Champion Health Patients will call Amaze (usually via Teams as they haven’t set up their Amaze account yet) and will want to unenroll from champion health. In an effort to be as helpful as possible, we will let the patient know we are happy to help them and then follow the below instructions. Make sure to always give the patient Champion Health direct line so they can call on their own behalf if desired

- Email: Bet1H2Henrollments.com, sending from Member Services (not personal email)
- Subject Line: “Champ Unenroll Request/s”
- Body: Provide (either typed out or screenshot from portal search) patient’s first and last name, email, phone #, and company
- Indicate whether it is an unenrollment or question about champ
